We specialize in:
- Home and Community Based Waiver Services
- Transition planning from school to adult services in the community
- In-home supports for aging caregivers who provide care for a family member with disabilities and special needs
- Services for consumers who have intellectual and related disabilities and autism.

Need a Ride?
Transportation for Medicaid recipients is available for doctor appointments, dialysis, x-rays, lab work, drug store or other medical appointments.
- To ask for a ride, call at least 3 days before your appointment.
- Call Monday – Friday 8:00am to 5:00pm
- To cancel a ride call at least 24 hours in advance.

Buddy Walk with The National Down Syndrome Society
